The Paediatric Diabetes Education Program is located within the Halton Diabetes Program at Oakville Trafalgar Memorial Hospital. The program provides family-centered diabetes care to children and teens aged 5-17 with type 1, type 2 or prediabetes.
Referral Process
A physician’s referral is required to access the Pediatric Diabetes Education Program. Primary care providers (Paediatrician, family doctor or nurse practitioner) can refer a patient by completing the Paediatric Diabetes Services Referral Form (PDF) and faxing it to the Halton Diabetes Program at 905-815-5101.
The program is not for emergencies. If your child needs to be seen on an urgent basis, please contact your family doctor or go to your local emergency department.
What to expect
Our team includes paediatric endocrinologists, nurses, dietitians, and social workers who work closely with children and their families to make a plan for managing the child’s diabetes. Our team works with children with newly diagnosed diabetes or those who have had diabetes for years. The goal of our program is to create a personalized care plan for each child and empower families to actively and effectively manage diabetes.
What to Bring
- Your child’s Health Card (OHIP)
- Any technology that your child uses to help with their diabetes management
- Any forms you need signed by the physician
- A list of your child’s current medications including over-the-counter medications, vitamins and herbal medications
- Blood sugar monitor (glucose meter or continuous glucose monitor)
- Log book or record of your child’s blood sugars
- Low sugar supplies
Cancelling or changing an appointment
Your appointment is reserved just for you and when it is cancelled with short notice, or missed altogether, it is often too late or impossible to offer this time to someone else and the appointment time goes unfilled.
If you need to cancel or change your appointment, please call 905-338-4437 ext. 2 to speak with a member of our team. Please call at least 24 hours before your scheduled appointment. If you miss your appointment without notifying us, we may discharge you from the program.
Late arrival
If you are late for your appointment, it may be shortened or rescheduled. This is to make sure we have enough time to help you with your healthcare concerns.
