The Navigator Service provides short term support and case management to children, youth and their families as they navigate through the mental health system and transition to longer term services. Services provided include connecting children, youth and their families to the most appropriate service provider, liaising with other parts of the health care system , accompanying clients to their appointments with psychiatrists and mental health workers, crisis intervention and outreach, in-school support to children and their teachers.
Children and youth up to the age of 17 years of age who are at risk for hospitalization, or who are transitioning from the inpatient setting or the Emergency Department to outpatient care.
How to Access This Service
Referrals are accepted from family physicians by faxing the Patient Referral Form to one-Link.
