Halton Diabetes Program

The Halton Diabetes Program provides care to children and adults with diabetes in the communities of Oakville, Burlington, Milton and Halton Hills. Our team supports patients learning knowledge and skill on diabetes management and staying on track with healthy living behaviours. We provide education through individual appointments, group programs, and workshops at a variety of locations in the Halton region.

A family member, significant other or friend is welcome to come with you to the program.

About the Program

Our team consists of diabetes educators (nurse educators and dietitian educators), a social worker, a kinesiologist (exercise specialist), endocrinologists (diabetes doctors), a nurse practitioner, and support staff. You and your family are important members of the team. Together we can set up goals and develop a plan of care to meet your needs.

We provide outpatient care, education and support to children and adults with:

  • Type 1 diabetes
  • Type 2 diabetes
  • Prediabetes
  • Diabetes and pregnancy

Adult Education Program

The Halton Diabetes Program offers a variety of services for adults (18+ years) with all types of diabetes and prediabetes in Oakville, Burlington, Milton and Halton Hills.

Referral Process

Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program.

Adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton Central Intake Program at 905-338-2983 ext. 4872 to initiate the referral process (self-referral). Once the self-referral has been initiated, your family doctor or other health care provider will be contacted to complete the process and the referral will be sent to our program. Our program will contact you directly to schedule an appointment once we receive your referral.

What to expect

We offer one-to-one teaching and group sessions with our educators. On your first visit, you may be scheduled to join a group education class or to see the diabetes educator for an initial assessment, which is based on the clinical information indicated on the referral.

  • One-to-one
  • Individual initial assessment appointment (in-person or Virtual): The diabetes educator will ask you questions to learn more about your health history and assess your need for education. A plan of care will be discussed and shared with you. A discussion about when your next visit and/or telephone call will also occur.
  • Follow up visits (in person/phone/video): The frequency of follow up appointments are booked depending on the initial assessment and your needs for education.

  • One-to-one appointment with Nurse Practitioner (Endocrinology) or Endocrinologist for patients who meets the Endocrinology Consult Criteria

What to Bring

  • Health card (OHIP)
  • A list of your current medications including over-the-counter medications, vitamins and herbal medications

  • Blood sugar monitor (glucose meter or continuous glucose monitor)

  • Log book or record of your blood sugars

  • Low sugar supplies

  • A person to translate for you if you need one

  • A family member or support person if you wish

Cancelling or changing an appointment

Your appointment is reserved just for you and when it is cancelled with short notice, or missed altogether, it is often too late or impossible to offer this time to someone else and the appointment time goes unfilled.

If you need to cancel or change your appointment, please call 905-338-4437 ext. 1 to speak with a member of our team. Please call at least 24 hours before your scheduled appointment. If you miss your appointment without notifying us, we may discharge you from the program.

Late arrival

If you are late for your appointment, it may be shortened or rescheduled. This is to make sure we have enough time to help you with your healthcare concerns.
