Mississauga Halton Diabetes Foot Care Program

The Mississauga Halton Diabetes Foot Care Program provides care to adults living with diabetes that require assistance with foot care management in the communities of Oakville, Milton, Halton Hills, and Mississauga.  

Our team supports patients in their foot care management journey. We provide care and self-management education through individual appointments, and access to virtual and in person foot care workshops. 

A family member, significant other or friend is welcome to come with you to your appointment. 

ABOUT THE PROGRAM 

OUR GOALS 

  • To improve access and remove barriers to foot care services for at risk patients living with diabetes 
  • To prevent diabetes foot complications  
  • To provide the education needed for individuals to care for and maintain healthy feet 

OUR SERVICES 

  • Routine basic and complex nail care 
  • Corn and callus reduction 
  • Ongoing diabetic foot assessments 
  • Footwear education 
  • Foot care and self-management education 

Healthy Feet Workshop (Partnership with the MH Self Management Program) 

  • Understand the impact of diabetes on your feet 
  • Discuss factors that lead to foot complications 
  • Learn how to prevent diabetes foot complications 
  • Available in person or virtual

OUR TEAM 

Foot Care Nurse (RPN) – A nurse with specialized foot care training.  

Chiropodist (DCh) – A foot and ankle specialist trained in the diagnosis and treatment of foot disorders.

HOW CAN I BE REFERRED?  

If you are currently enrolled in Diabetes Education, Home and Community Care (Wound), and/or Renal Program (Dialysis) your healthcare provider can directly refer you into the Foot Care Program based on eligibility Criteria.   

If you are NOT currently in any above programs Your healthcare provider can refer you to a 

  • Diabetes Education Program. Through this program, you will receive a one-on-one assessment with a Diabetes Educator who can refer you to the Foot Care programs if you meet the Eligibility Criteria. 
  • You can start a self initiated referral into a Diabetes Education Program.  
    • Once the self-referral has been initiated, your family doctor or other health care provider will be contacted to complete the referral process.  
    • You will be contacted directly to schedule an appointment from a Diabetes Education Program.  
    • The Diabetes Education Program will then refer you to the Central Intake Program for processing to determine your eligibility for the Halton Diabetes Footcare Program. 
  • All referrals are submitted to the Central Intake Program for processing. Learn more about the Central Intake Program.

WHAT IF I NEED TO MAKE CHANGES TO MY APPOINTMENT? 

Your appointment is reserved just for you and when it is cancelled with short notice, or missed altogether, it is often too late or impossible to offer this time to someone else and the appointment time goes unfilled. 

To ensure a smooth and efficient appointment, kindly bring the following items: 

  • Your current Health Card  
  • Medication list 
  • Medical history
  • Initial visit paperwork

Reschedule or cancel your appointment: 

  • Call 905-338-4802 at least 24 hours prior to your appointment time to cancel or reschedule your appointment. This helps us accommodate other patients efficiently. 

No Reminder Call:  

  • A reminder call is no longer offered, please make sure you have your appointment marked on your calendar or set a reminder. 

Late arrival 

  • If you are late for your appointment, it may be shortened or rescheduled. This is to make sure we have enough time to help you with your healthcare concerns. 

No Show or Frequent Cancellations 

  • Given the extended wait list for our program, it is imperative that you attend your scheduled appointment. Failure to do so may result in discharge from the program.
