The Remote Care Management Program realizes the potential of technological monitoring services to improve the patient experience by allowing patients to heal and recover at home, while being closely monitored by the Remote Care Management team.
Remote Care Management realizes the potential of technology to allow patients to heal and recover at home while having vital signs reviewed remotely by the Remote Care Management team.
The program is open to patients with COVID-19, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF) and other respiratory illnesses (e.g. pneumonia) that would benefit from monitoring and home care and/or support services in the community.
Before discharge from hospital each patient meets with their healthcare team to review their care plan and to set-up enhanced support services for them at home, which may include nursing, personal care workers, physiotherapy, or social work.
Patients enrolled in Remote Care Management will receive a loaned Halton Healthcare monitoring kit. The kit contains a tablet, charger, stand and Bluetooth enabled equipment which may include pulse oximeter, blood pressure monitor and/or scale.
Patients are monitored by the Halton Healthcare Care Management team, Monday to Friday, 9:00 am – 5:00 pm, excluding holidays. They have access to Home and Community Care Support Services from a rehabilitative perspective and an additional 24/7 Access to Care Line. C.AIR, our home oxygen provider, conducts home visits to assess/reassess their needs for ongoing oxygen therapy if required. If patients require urgent medical assessment, this will be performed by C.AIR Respiratory Therapists in their homes or after-hours by Halton or Peel Community Paramedics. Medical clarification/questions are directed to the patient’s primary care provider as they continue to be the Most Responsible Provider for their patients.
