Complex Transitional Care Unit

The Complex Transitional Care Units at our three hospitals all provide care to patients who require extensive assistance with activities of daily living and frequently have complex medical, functional/cognitive problems, and end of life care needs.

The interdisciplinary team members work diligently to maintain or improve patient functioning to optimize quality of life. The unit’s case mix includes patients with:

  • dementia,
  • chronic obstructive pulmonary disease
  • multiple sclerosis
  • cerebral aneurysm, acquired brain injury
  • diabetes
  • depression
  • Parkinson’s syndrome
  • cardiac conditions and skin and
  • wound care issues

The Therapy team which includes Nursing, Occupational Therapy, Physiotherapy, Speech-Language Pathology, Dietician, Social Work, and Recreational Therapy, provides support and activation/stimulation programs.

Palliative Care is a program offered within the Complex Continuing Care Program. The goal of palliative care is to provide whole-person health care that aims to relieve suffering and improve the quality of living and dying.
