Patient and Family Advisory Committee Terms of Reference


To identify opportunities to improve the care experience for patients, family, and caregivers.


  • To listen and learn from patients, family members and/or their caregivers to improve on the patient experience.
  • To identify ideas to improve patient experiences through sharing and reflection on personal experiences with the health services at Halton Healthcare.
  • To foster and maintain open communication between the healthcare team and patients/families to enable the opportunity to provide recommendations and insights based on the patient, family, and caregiver experience.
  • To advise on ways to enhance the partnership with patients, family members and/or caregivers of patients.
  • To provide recommendations for improvement.


  • The membership will consist of 4-6 patients, family members of patients and/or caregivers who have received services through Halton Healthcare in the last 2 years.
  • The committee will also include up to 3-4 staff members invited to sit on the Committee.  The staff will represent a cross section of the organization and may include a social worker, charge nurse, manager.


  • The Committee is co-chaired by:
    • A patient/family member/caregiver 
    • A staff member
  • The Chairs will be jointly responsible for developing the agenda; chairing the meetings; as well as communicating with members of the Committee.

Meeting Etiquette

  • Regularly attend and be active participants.
  • Treat every participant with respect and courtesy.
  • Allow every participant to be heard, without interruption, and to have their needs addressed.
  • Keep discussions focused, concise, and confidential (not identifying patient names, or sharing details outside of meeting).

Frequency of meetings

  • Meetings will be held every two months. 
  • Ad hoc meetings, if required, to be called by the chair.
  • The location of the meetings will be the Burlington Dialysis Center.

Meeting Minutes

  • Minutes of the meetings will be recorded and available for all committee members.
  • They will be posted in the patient lounge and on the Nephrology PFAC webpage.
  • Minutes will reflect membership as invited and attended.
  • Guests are recorded in the minutes as non-voting.


A quorum for PFAC is 50% plus 1 of rostered members, and one of the co-chairs.
