Community Care Access Centre (CCAC)

Your Community Care Access Centre (CCAC) is committed to coordinating the delivery of high quality in-home and community-based care. CCAC employees are experienced and knowledgeable about local community support services, information and referral and long-term care placement. They will help you and your loved ones understand the local health system and access the services needed to live independently in the community.

Hospital patients may be assessed by on-site CCAC Case Managers prior to discharge to determine their needs once they leave the hospital.

What the CCAC does:

  • Connect people of all ages to the health and community care they need.
  • Provide healthcare at home, at school and in the community where appropriate.
  • Help clients maintain independence in their own homes with dignity.
  • Find the right long-term care home when living independently is no longer possible.

CCACs are for everyone. Their services are funded through the Ministry of Health and Long-Term Care at no direct cost to clients.

Note:  Anyone can make a referral to CCAC. Physician signature only required for Nursing and Physiotherapy Weight Bearing.  Download referral form now.
